Thank you for contacting us and submitting a request to our Service Desk.
Unfortunately only the standard fields available by default, excluding custom fields, can be added to an Enquiry Form, however I can see how the custom inclusion of standard fields this could be useful and encourage you to submit an idea to our iSAMS Ideas Portal. This area is regularly reviewed by our Product and Development teams, any ideas suggested are reviewed/implemented based on demand & feasibility.
